New patient appointments are offered on a regular basis at the practice. However, due to significant demand please note that we currently are unable to accomodate any new patients to the practice. We thank you for your interest, and request you to please check again in a few months for updates.
Ontario Family Practice require all new patients to complete and submit the Patient Registration Form prior to their appointment.
Please arrive at least 15 minutes prior to your scheduled appointment time so that you can complete the required registration paperwork.
You will have an initial assessment with the nurse prior to your interview wiht your doctor.
New patients will also need to transfer their medical records to this practice. To do this, complete the Transfer Medical Records form at the front desk and we will transfer medical records from your former practice.
We are currently unable to accommodate new patients, but new family members (newborns, spouses/partners) of current patients will be accepted.
